- Location: Clintonville and New London region
- The team consists of 6 Physicians and 10 APCs
- 2-3 SNFs a day (patient schedule built within EPIC)
- On-Call by phone only: 1:11
- 4 weekends per year, Physicians cover holidays 1 per year, back up call by week (just for support for APC if needed), no going in when on call
- Patient census: 8-12
- Support staff includes RNs and schedulers who help to coordinate provider visits, communicate with facilities, and triage phone calls.
- 27 Facilities total between skilled nursing and assisted living facilities
- Large opportunities for growth, such as Medical Director roles in the nursing homes that we serve
Senior Care Services:- Visits completed at patient's location (Nursing Home or Assisted Living)
- Acute visits, compliance visits, & discharge visits for short and long-term residents
- Advance Care Planning & Goals of Care discussions
- Minor procedures at bedside
