Facility City & State California Facility Type FQHC Provider Profession Needed Dentist Specialty Dentistry Start Date 2026-11-09 End Date 2027-04-30 Length of Assignment 3 - 6 months - Ongoing Credentialing Timeframe 30 days Work Schedule Monday through Friday 8am-5pm. Might have flexibility to provide 4x10 at the Fortuna Community location but waiting on confirmation. Patient Volume Patient Volume: Average of 16-20 patients per day. MUST see 8 minimum patients per 4 hours. Procedures Bread and butter Dentistry License Needed CA License Notes Must have CA license and CA DEA Certifications Needed CV Relevant Experience/Skill Set Must have previous FQHC Dentist experience
