With the current environment surrounding COVID-19, more clients and their families are self-isolating, and many seniors are left without their typical caregivers and family members. We are looking for passionate caregivers to play integral roles in helping our community move forward. Your safety is the uppermost priority, and you will be able to provide care in a safe environment with appropriate PPE and measurements to maintain a safe environment.
As an LPN you are enthusiastic about Making Lives Better and promote Happier Aging. You are passionate about bringing peace of mind to your client's life and their families. You work closely with your Care Designer to create a premium experience and deliver on client care goals.
Duties and Responsibilities
Excellent customer service skills and ability to deal tactfully with clients, other interdisciplinary team members
As a brand ambassador the LPN treats client and team members with courtesy and respect
Reviews the care plans arranged by the case manager (Care Designer) prior to beginning a visit, completing records accurately and carrying out all assignments as requested
Responds to Care Services Center (CSC) Team regarding scheduling inquiries and updates within the agreed upon timeline
Engages in open communication with your Care Designer when opportunities to elevate your client's experience and care are identified
Delivers comprehensive professional nursing care consistent with the care plan, nursing assessments and client needs. The licensed practical nurse shall report changes in the patient's condition to the RN, and these reports shall be documented in the clinical notes.
Perform all skills, treatments and procedures competently in accordance to the provincial scope of LPN per the State of Illinois Nurse Practice Act (225 ICLS 65) and adheres to the policies, procedures and protocols of Nurse Next Door.
Assess and provide client and family/caregiver education and information pertinent to diagnosis.
Assists clients in all activities of daily living/hygiene assistance, bathing, grooming, linen changes and so forth.
Requirements and Experience
Current Certificate of Competence and in good standing with the provincial College of Nurses
Graduation from a recognized program for Practical Nurses and one year of recent related experience in gerontological nursing
Functions within the limits of own experience and knowledge, and practices safely and competently within the job description.
1 year of nursing experience required; three years nursing experience preferred. Community, home health or medical/surgical experience preferred.
Criminal Record Check with a Vulnerable Sector Search (updated at the nurses cost every 5 years for continued employment)
Negative TB test or chest x-ray (updated at the nurses cost every 5 years for continued employment)
Assists clients in all activities of daily living/hygiene assistance, bathing, grooming, linen changes and so forth.
Availability
Flexibility is required
Casual / Part time
Must be flexible to be on call nights and weekends
Business Relationships
Reports to the Care Designer
Works closely with the Client Care Ambassador
