About the Role:

The Nurse Practitioner will serve as a vital healthcare provider delivering comprehensive family medicine services across diverse patient populations, including pediatrics, geriatrics, and individuals experiencing pain or trauma. This role focuses on conducting thorough clinical assessments to diagnose and manage acute and chronic conditions while prioritizing patient safety and quality care. The Nurse Practitioner will collaborate closely with interdisciplinary teams to develop and implement individualized treatment plans that address both physical and psychosocial aspects of health. By integrating evidence-based pain management strategies and trauma-informed care, the practitioner will enhance patient outcomes and support long-term wellness. Ultimately, this position aims to improve access to high-quality healthcare services within the community, ensuring compassionate and effective care for all patients served.

Minimum Qualifications:

  • Current and unrestricted Nurse Practitioner license in the United States.
  • Master's or Doctoral degree in Nursing from an accredited program.
  • Certification as a Family Nurse Practitioner (FNP) or equivalent.
  • Demonstrated experience in clinical assessments, family medicine, and pain management.
  • Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
  • DEA License by hire date

Preferred Qualifications:

  • Experience working with geriatric and pediatric patient populations.
  • Additional certification or training in trauma care or emergency nursing.
  • Familiarity with electronic health record (EHR) systems and healthcare informatics.
  • Experience in patient safety initiatives and quality improvement projects.
  • Strong communication and interdisciplinary collaboration skills.

Responsibilities:

  • Perform comprehensive clinical assessments and physical examinations for patients of all ages, including pediatric and geriatric populations.
  • Diagnose and manage acute and chronic illnesses, with a focus on emergency medicine and chronic disease management.
  • Develop, implement, and monitor individualized treatment plans, ensuring adherence to best practices and patient safety protocols.
  • Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care and optimize health outcomes.
  • Provide patient education on disease prevention, health maintenance, and management of chronic conditions.
  • Document patient encounters accurately and maintain up-to-date medical records in compliance with regulatory standards.
  • Respond effectively to trauma cases, applying nursing and clinical expertise to stabilize and treat patients.
  • Participate in quality improvement initiatives and stay current with advances in clinical practice and healthcare regulations.

Skills:

The Nurse Practitioner will utilize clinical assessment skills daily to evaluate patient health status and identify medical conditions accurately. Expertise in family medicine and pain management will guide the development of effective treatment plans tailored to individual patient needs. Knowledge of geriatrics and pediatrics ensures appropriate care across the lifespan, addressing unique physiological and developmental considerations. Nursing skills, including trauma response and patient safety protocols, are critical for managing emergencies and minimizing risks during care delivery. Additionally, strong communication and collaboration skills facilitate teamwork with healthcare professionals and support patient education, fostering a holistic approach to health and wellness.
