About the Role
We're looking for a compassionate Nurse Practitioner or Physician Assistant to join our primary care team. You'll assess, diagnose and manage both acute and chronic conditions for about 20-25 patients per day. The role centers on preventive care, treatment planning and medication management. The ideal candidate would be an experienced provider (although new graduates with a strong background as an RN or EMT would be considered). Competitive compensation (bonuses), hourly salary (Depending Upon Experience) and benefits.
What You'll Do
- Conduct patient assessments and physical examinations
- Diagnose illnesses and develop individualized treatment plans
- Manage medications, including adjustments for patients with multiple prescriptions
- Manage chronic conditions such as diabetes, hypertension, high cholesterol and thyroid disease
- Provide preventive care, annual wellness exams and health education
- Counsel patients on lifestyle and health decisions.
- Coordinate care with physicians, nursing staff and specialists
- Keep accurate, timely documentation in the EMR
Qualifications
- Active Texas license as a Nurse Practitioner (APRN) or Physician Assistant
- National board certification (AANP/ANCC for NPs, NCCPA for PAs)
- Master's degree from an accredited NP or PA program
- At least 1 year of primary care experience preferred
- Strong clinical assessment and pharmacology knowledge
- Experience with chronic disease management and geriatric patients
- Proficiency with EHR systems
- Excellent communication and counseling skills, with empathy and sound judgment
