geriatric-focused primary care in the
San Fernando Valley, California. This role is ideal for providers who enjoy
autonomous practice, relationship-based care, and a predictable weekday schedule.
The position is
home-based, with providers seeing patients in
residential homes and care facilities, supported by a structured clinical model and virtual physician oversight.

Schedule & Workload•
Monday Friday•
Daytime hours•
No nights, weekends, or call• Average
5 7 patients per day• Assigned patient panel once ramped (approximately patients)
Clinical Environment• Home-based and facility-based geriatric care
• Focus on chronic disease management, preventative care, and continuity
• EMR system in place
• Physician support available virtually/by phone
• Structured workflows designed for quality over volume
Candidate Requirements• Active
California NP license• Board Certified Nurse Practitioner
•
DEA and BLS required
• Minimum
1 year of experience in geriatrics, primary care, home health, or similar setting
• Comfort working independently in a mobile/home-based model
• Clean professional and licensure history
