Key Responsibilities- Provide primary care for frail and elderly residents across an assigned group of nursing homes
- Lead advance care planning conversations, including POLST
- Coordinate care with families, facility teams, and specialists
Qualifications- Board-certified NP or PA with a DEA
- Geriatric experience, ideally 3+ years
- Adult/Geriatric NP certification, long-term care, or skilled nursing experience a plus
Compensation & Benefits- $140,000 to $155,000 annually, plus productivity bonus for visits above 260 per month
