We are seeking a compassionate and experienced Nurse Practitioner (NP) to provide high-quality primary care services to residents in assisted living communities throughout the Albany region of New York.

Position Options Part-Time Opportunity
- 2 3 full days per week
- Travel between Hoosick Falls, NY and Cambridge, NY
Full-Time Opportunity
- 4 5 full days per week
- Travel between Hoosick Falls, NY , Cambridge, NY , and Troy, NY
What We Offer

- Expected start date within 4 6 weeks, pending credentialing and onboarding
Required Qualifications
- Active New York State Nurse Practitioner or Physician (MD/DO) license
- Active DEA registration
- Currently enrolled and credentialed with Medicare and Medicaid (required)
- Ability to practice independently
- NPs must meet New York State requirements, including a minimum of 3,600 clinical practice hours
- Primary care experience required
Preferred Qualifications
- Geriatric care experience
- Assisted living, long-term care, or skilled nursing experience
