Locum tenens opportunity for Nurse Practitioners and Physician Assistants to perform home-based health assessments across New York in a structured, non-treatment setting.

• Flexible scheduling
• Minimum 9 hours of availability per week

• Nurse Practitioner or Physician Assistant
• Active New York license
• NY Medicaid ID required
• NP: Minimum 3,600 hours of experience
• PA: Supervising physician assigned
• COVID vaccination required
• Recent physical (within 12 months)

• Conduct in-home health assessments for patients
• Complete structured evaluations (non-diagnostic, non-treatment role)
• Ask standardized assessment questions and document responses
• Review patient history and current health status
• Submit completed documentation in a timely manner

• 90 minutes per visit total:
  • 30 minutes: travel prep, patient outreach, chart review
  • 30 minutes: in-home assessment
  • 30 minutes: documentation and submission

• Independent, field-based role
• No prescribing, treatment, or procedures required
• Focus on health assessments and documentation
• Laptop provided for documentation
