Pay Range $ 28 / per hour - $ 35 / per hour

What You'll Do - Provide direct patient care in the home
- Monitor patient condition and report changes
- Administer medications and treatments as ordered
- Support individualized care plans
- Document care and communicate with the healthcare team
Required: - Active LPN license
- Current BLS certification
