Registered Nurse (RN) - Rehab Responsibilities: Manage and coordinate total patient care in the rehabilitation unit before, during, and after therapeutic treatments. Serve as the primary patient advocate throughout the recovery process, ensuring their physical safety, emotional well-being, and privacy. Prepare the rehabilitation environment by ensuring all required medical, mobility, and adaptive equipment is available, safe, and functional. Verify patient identity, medical history, and rehabilitation goals according to established hospital safety protocols and intake assessments. Maintain strict infection control protocols and continuously monitor the unit for potential safety or fall hazards. Perform accurate assessments of patient functional status, skin integrity, and pain levels in collaboration with therapists before and after daily sessions. Monitor and record patient vital signs, physiological status, and progress toward therapeutic milestones in the electronic medical record. Anticipate the needs of the care team, efficiently coordinating with physical, occupational, and speech therapists to deliver seamless, integrated care. Safely position, transfer, and mobilize patients to protect skin integrity, prevent contractures, and encourage proper body alignment. Provide a comprehensive hand-off report to discharge coordinators, home health agencies, or outpatient teams to ensure a safe transition of care. Participate in emergency procedures and manage rapid changes in patient condition during critical medical or cardiovascular events.
