Overview In this role you ensure accurate review and approval of patient information for care starts and ongoing certifications, supporting regulatory compliance. You partner with branch leadership to educate and train Licensed Professionals, maintaining strong relationships with referral sources, patients, and caregivers. You promote high-quality patient care and staff understanding of documentation requirements. This position combines clinical oversight with education to sustain compliant, effective home health/hospice services. You will contribute to Enhabit's mission by shaping documentation practices and staff development in a collaborative setting.

Responsibilities- Review and approve patient information submitted by Licensed Professionals during start of care, recertification, resumption of care, or evaluation visits
- Partner with branch leadership to provide ongoing education and training on documentation requirements to meet regulatory standards
- Conduct one-on-one trainings when needed to deepen staff understanding of processes
- Promote quality patient care and maintain stable relationships with referral sources, patients, caregivers and employees
Key requirements- Graduate of an approved nursing or physical therapy program
- Valid clinical license and ability to maintain per state requirements
- Minimum of one year of experience (Medicare home health or hospice preferred)
- CPR certification
- Valid state driver license and automobile liability insurance
- Basic technology skills and experience with mobile devices
- Preferred: experience with packet review/quality improvement; coding experience; familiarity with Homecare Homebase (HCHB)
- Preferred: intermediate Microsoft Office skills (Word, Excel)
