Overview In this role you act as a local member advocate, using community knowledge to support vulnerable members in navigating health care and available services. You work with the health care services team to provide non-clinical support in the field and in members' communities. You help members maximize benefits, arrange care, and connect them with social resources. Your work contributes to delivering quality, cost-effective member care while expanding outreach to hard-to-reach populations. This is a people-centred, hands-on role with meaningful impact on members' health journeys.

Responsibilities- Engage with members as an advocate and resource to support management of health care needs
- Collaborate with and support the health care services team with non-clinical paraprofessional duties in the field (homes, shelters, provider offices)
- Assist members in navigating and maximizing health plan benefits (appointments, transportation, prescriptions, follow-ups)
- Help members access social services and community resources (housing, food, employment)
- Provide outreach to locate/provide support for disconnected members with special needs
- Conduct research to locate unreachable members using databases, providers, or community resources
- Participate in ongoing or project-based outreach activities (telephonic and/or face-to-face)
- Guide members to maintain Medicaid eligibility and other financial resources
Key requirements- At least 1 year of health care experience with underserved or special needs populations or equivalent experience
- CHW certification may be required in some states
- Valid driver's license, reliable transportation, adequate auto insurance for job-related travel
- Ability to multi-task while speaking with members
- Excellent customer service, organizational and time-management skills
- Willingness to learn other lines of business, programs and software systems
- Excellent verbal and written communication skills
- Proficiency in Microsoft Office and applicable software programs
