Family Practice/Primary Care Physician Assistant

We are currently working with HarmonyCares Medical Group. They are seeking NP/PAs to do in Home Health & Telehealth visits in Hinds County, MS. After reviewing the details below, please let me know if you would like to be considered for this opportunity.

Cities: Jackson, Raymond, Clinton, Utica, Byram, Terry, Edwards, Bolton, Learned

NP/PA Home Health (HRA s):
• Seeking coverage starting ASAP
• Schedule: Monday Friday (will consider 3 days but prefer all 5)
- Hours: 8a-5p
• 70% Telehealth 30% in Home
• Must be comfortable seeing all ages
• The schedule is made 2-3 weeks in advance
• Performing 5-7 HRA's per day (possibly more depending on need)
• Perform annual risk assessments and review of chronic conditions
• Assessment Only Role
- No treatment or prescribing, no H&P, no patient follows up, no chronic disease management
• Equipment is provided: iPad, Blood Pressure cuff, Lab kits
• $85 per assessment
