- Active NCCPA certification

CREDENTIALING REQUIREMENTS
- MMR and Varicella documentation or positive immunity titers
- Two-step TB test within the past 12 months
- QuantiFERON Gold accepted per facility requirements
- If TB positive: chest X-ray within the past 3 months
- Case logs from the previous 2 years
- Preeclampsia-Eclampsia CME required upon initial appointment and every 2 years for reappointment
- Must meet all facility credentialing and privileging requirements
