Experience can be demonstrated in one of the following ways: 1)Applicants who have just completed a residency shall provide a copy of their residency log and/or documentation from their program director attesting that the applicant meets the above criteria. 2)Applicants who are not applying directly out of a residency shall provide quality profiles from hospital(s) or clinic(s) where they currently have privileges, showing clinical activity for the last 24 months, including number of procedures performed, as applicable.Will provider work solo?
