What You ll Do:
- Provide outpatient medical care to adults aged 55+ in a PACE program
- Conduct home and community visits alongside interdisciplinary teams
- Manage patient care using electronic medical records (EMR)
- Collaborate with nurses and other healthcare professionals to coordinate visits
- Follow clinical protocols for geriatric health management
- Assist with intake and ongoing care for elderly patients
- Participate in team meetings and community outreach activities
What You Need:
Required: - Active Medical License and DEA
- Board Certified or Truly BE in Internal or Family Medicine
- BLS certification
- Geriatric care experience preferred
Preferred: - Experience with PACE programs or home/community-based elder care
- Strong communication and teamwork skills
