We are seeking a full-timeAdult Primary Care Physician to provide comprehensive medical care to membersenrolled in an Institutional Special Needs Plan (ISNP). This role offers theopportunity to make a meaningful impact on medically complex seniors throughproactive care management, interdisciplinary collaboration, and a strong focuson improving outcomes and preventing avoidable hospitalizations. Qualifications
- MD or DO degree from anaccredited medical school.
- Completion of an accreditedFamily Medicine, Internal Medicine, or Geriatrics residency program.
- Board Certified or BoardEligible.
- Active Nevada medicallicense, or ability to obtain licensure.
- Valid DEA registration.
- Two or more years ofexperience in SNF, LTC, post-acute care, or complex senior care preferred.
- Experience with HCC coding,Medicare Advantage, HEDIS, and STARs documentation preferred.
- Comfortable practicing ininstitutional and post-acute care settings.
- Ability and willingness totravel between healthcare facilities in Nevada.
