In this role,you will bring high-quality, relationship-based care directly to patientsthrough home visits, community-based care, and virtual appointments whileworking within a patient-centered, value-based care organization focused onimproving outcomes and supporting long-term health.

You will enjoy a high level ofclinical autonomy while working alongside an interdisciplinary team dedicatedto helping patients achieve better health outcomes.
Position Highlights
- Full-Time Schedule:Monday-Friday 8:00 AM-5:00 PM
- Competitive Base Salary: $130,159-$195,238

Qualifications
- Active and unrestrictedCalifornia Nurse Practitioner license
- Active DEA license
- Current BLS certification
- Strong clinical assessmentand diagnostic skills
- Ability to practiceindependently while collaborating effectively with an interdisciplinary careteam
- Commitment topatient-centered, preventive, and value-based care
- Excellent communication andrelationship-building skills
- Comfortable providing carein home, community, and virtual settings
