Continuum Therapy Partners is seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team at North Shore Healthcare at Marshfield in Marshfield, Wisconsin.
This full-time Physical Therapist opportunity offers the chance to provide high-quality physical therapy services in a supportive, collaborative skilled nursing facility where your clinical expertise makes a meaningful impact every day.
Join an experienced rehabilitation team providing patient-centered care while working alongside Occupational Therapists, Speech-Language Pathologists, Physical Therapist Assistants, and other healthcare professionals. Treat primarily adult and geriatric patients while helping them improve mobility, strength, balance, endurance, and functional independence.
Responsibilities
Evaluate patients and develop individualized physical therapy treatment plans.
Provide skilled physical therapy interventions to improve mobility, strength, balance, and independence.
Perform therapeutic exercises, gait training, mobility training, and functional activities.
Monitor patient progress and adjust treatment plans based on patient needs.
Collaborate with physicians, nursing staff, caregivers, and interdisciplinary rehabilitation team members.
Complete accurate and timely documentation according to company, state, and federal guidelines.
Educate patients and families on mobility strategies, exercise programs, and safety techniques.
Provide compassionate, evidence-based physical therapy services.
Qualifications
Graduate of an accredited Physical Therapy program.
Current Wisconsin Physical Therapist (PT) license required.
Skilled nursing or rehabilitation experience preferred but not required.
New graduates are encouraged to apply.
Strong communication, organization, and teamwork skills.
