Field Nurse Practitioner (Home & Community-Based Care) We are seeking a Field Nurse Practitioner to provide preventive and primary care services directly in the communities where patients live. This role focuses on in-home visits, skilled nursing facilities, and community-based pop-up clinics, delivering care to patients who may face barriers to accessing traditional healthcare settings.

The Nurse Practitioner will perform comprehensive medical assessments and preventive screenings for patients across a variety of settings. Visits are focused on initial assessments and identifying gaps in care, rather than ongoing follow-up treatment.
Providers work primarily in non-clinical environments, traveling within an assigned territory to meet patients where they are most comfortable
