  • Schedule: Outpatient clinic, M-F, 8am-5pm (considering 3-4 days/week)
  • Duration: At least 2 weeks per month (prefer full-time availability)

  • Licensure: Active medical license, DEA required
  • Experience: Geriatric experience preferred, BC or BE in Internal or Family Medicine
  • Certifications: BLS required

What You ll Do
  • Provide outpatient primary care to adults over 55 in clinic and community settings
  • Conduct home visits to assisted living, SNFs, or private residences with the support of an interdisciplinary team
  • Manage a predominantly geriatric patient panel, including chronic disease management and preventive care
  • Collaborate with a team of nurses and other healthcare professionals to coordinate patient care
  • Utilize electronic medical records (EMR) for patient documentation and scheduling
  • Support PACE program initiatives to deliver comprehensive care for the elderly
What You Need
  • Active medical license and DEA registration
  • BC or BE in Family Medicine or Internal Medicine
  • Geriatric care experience preferred
  • BLS certification
  • Excellent communication skills and compassionate bedside manner
  • Ability to work independently and as part of a multidisciplinary team
