We are seeking Nurse Practitioners and Physician Assistants for flexible locum tenens opportunities conducting in-home health assessments throughout West Virginia. This position offers flexible scheduling with both short-term and long-term assignment options available.

No nights or weekend requirements
Schedule
Flexible scheduling Monday Sunday
Daytime hours between 7a 7p
Providers manage their own schedules within available coverage needs
Practice Setting
Home Health
In-home evaluations for Medicare Advantage, Medicaid, and commercial health plan members
Mobile community-based care model
mySignify EMR platform
Clinical Responsibilities
Providers will conduct pre-scheduled in-home health evaluations, including:
Medication reconciliation and medical history review
Comprehensive health assessments and physical evaluations
Documentation utilizing patient-specific pre-populated forms
This role does not include:
Patient Volume
Required Qualifications
Active NP or PA license required
Board certification required
Master s degree required

Coverage available across multiple counties throughout West Virginia
Travel assistance may be available depending on eligibility
Credentialing timeframe approximately 1 3 weeks
