Position Overview
A large multispecialty group in New York City is seeking Nurse Practitioners (NPs) and Physician Assistants (PAs) to provide high-quality, patient-centered care in patients' homes and nursing facilities. This role is ideal for providers who enjoy holistic care, chronic disease management, and direct patient engagement.

Providers have the option to work in Brooklyn, Manhattan, or The Bronx, offering flexibility based on geographic preference.
Key ResponsibilitiesPatient Care & Evaluation
Conduct comprehensive patient assessments, including physical exams, medication reconciliation, and medical history review during home or nursing facility visits
Diagnose medical conditions and develop individualized treatment plans
Prescribe medications and manage acute and chronic illnesses within scope of practice
Care Coordination & Collaboration
Collaborate with physicians, specialists, and multidisciplinary teams to ensure continuity of care
Provide referrals and coordinate follow-up with other healthcare providers
Communicate patient needs and clinical updates with the care team
Patient & Family Education
Educate patients and families on disease management, lifestyle modifications, medication adherence, and preventive care
Answer patient and family questions and provide guidance regarding treatment plans

QualificationsNurse Practitioner
Master's Degree from an accredited NP program
Family, Adult-Gerontology, or related NP focus preferred
Current New York State NP license
National Board Certification through AANP, ANCC, or applicable certifying organization

Physician Assistant
Master's Degree from an accredited Physician Assistant program
Current New York State Physician Assistant license
NCCPA certification

Due to the patient population, the client is seeking providers who are bilingual in Russian or Chinese
Experience with EMR/EHR systems preferred
Strong clinical judgment and communication skills
Ability to work independently while collaborating effectively with physicians and multidisciplinary teams
Willingness to provide care in patients' homes and nursing facilities
Compensation & Benefits
Competitive salary: $135,000 - $150,000 annually
