Requirements:
  • Board Eligible or Board Certified in Otolaryngology - REQUIRED
  • Current or Willing to obtain Colorado Medical License - REQUIRED
  • Interest in practicing General ENT full-time - REQUIRED
  • Availability/start date - REQUIRED
  • Clean malpractice/license history - HIGHLY PREFERRED
  • License history and disciplinary history - REQUIRED
  • Candidate's subspecialty interests (Laryngology, Skull Base Surgery, etc.) - HIGHLY PREFERRED
  • Clean malpractice/license history - HIGHLY PREFERRED
  • Most recent NPDB Self Query Report (within 30 days active) - REQUIRED
  • DEA (must be obtained prior to start) - REQUIRED
  • Visa Support: Does provider require J1 or H1B support? (note in "Additional Submission Details" field)- REQUIRED
