Job Summary: We are seeking a
friendly and compassionate Primary Care Nurse Practitioner/ Physician Assistant to join our growing team. In this role, you will provide comprehensive Primary Care care to patients in post-acute care settings while collaborating closely with physicians, nursing staff, and interdisciplinary care teams. Responsibilities: - Conduct Primary Care assessments for patients in post-acute care settings
- Evaluate, diagnose, and manage Primary Care conditions including COPD, asthma, pneumonia, and respiratory failure
- Develop and implement individualized treatment plans
- Collaborate with interdisciplinary teams
- Educate patients, families, and facility staff on respiratory care and treatment plans
- Monitor patient progress and adjust care plans as needed
- Maintain accurate and timely EMR documentation
- Support initiatives focused on reducing hospitalizations and Return-to-Hospital (RTH) events
Requirements - Preferred a minimum of 6 months of experience in post-acute care setting
- Preferred experience in Primary Care: training provided
- Active and unrestricted state Nurse Practitioner license or Active PA license and NCCPA certification required
- Master s Degree in Nursing from an accredited Nurse Practitioner program or Master s Degree from an accredited Physician Assistant program required.
- Strong communication, organizational, and teamwork skills
- Ability to work independently in the clinical environment
