We are a growing, physician-owned medical group expanding in Batavia , IL seeking an experienced Nurse Practitioner to join our internal medicine team in a Skilled Nursing Facility (SNF) setting.
Key Responsibilities: - Provide comprehensive medical care to SNF patients, including assessments, diagnosis, and treatment planning
- Responsible for full medical management and primary care
- Ensure accurate and timely documentation in EMR
Required Qualifications: - Active Nurse Practitioner license in Illinois
- Active DEA license (or ability to obtain prior to start)
- Minimum 1 year of internal medicine experience as a NP
- Experience managing adult/geriatric patient populations
- Strong clinical decision-making and time management skills
- Comfortable working independently in an SNF setting
Preferred Experience:
- Skilled Nursing Facility (SNF) or post-acute care experience
- Background in internal medicine, geriatrics or inpatient hospitalist
Schedule & Setting:
- Weekends only (Saturday/Sunday)
- Skilled Nursing Facility Setting

Physical Requirements:
- Prolonged periods of standing, walking, and sitting while working with patients as well as working on a computer.
- Must be able to lift up to 50 pounds.
- May be exposed to bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.
- Must wear the required PPE per company and/or facility requirements.
- Must comply with facility vaccination requirements.
