Baptist AgeWell Center for Health is seeking a compassionate and experienced Geriatric Physician to join our growing senior-focused primary care team in Clay County, Florida. As part of Northeast Florida's most preferred health system, you'll practice in a collaborative, physician-led environment with extensive support services that allow you to focus on delivering exceptional patient care.
Position Highlights
- Full-time, outpatient senior primary care practice
- Monday through Friday schedule, 8:00 AM - 5:00 PM
- Minimal call responsibilities
- Lead a collaborative care team alongside experienced Advanced Practice Providers
- Epic EMR

Qualifications
- MD or DO
- Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Active or eligible Florida medical license
- Passion for senior care and population health
- Minimum of 5 years of experience in Internal Medicine, Family Medicine, Hospital Medicine, or related practice with senior patients
- Geriatrics experience preferred
