- Clinical Scope:
- Provide in-home primary care to a medically frail, elderly population - Collaborate within a team-based care model including physicians, advanced practice professionals, RN care managers, social workers, and medical assistants - Serve approximately 830 patients across a 10-county area in West Michigan - Additional Responsibilities:

Qualifications- Board Eligible/Board Certified in Geriatrics or Internal Medicine with geriatrics experience
- Medical Degree (MD/DO) with licensure in Michigan
- Certification in Basic Life Support (BLS)
