IJN Health Systems, LLC, a Skilled Home Health Agency, is looking for a Licensed Practical Nurse (LPN) to join our team of strong and compassionate caregivers. Our ideal candidate is a self-starter, motivated, and engaged.
Licensed Practical Nurse (LPN/ LVN) -Home Health Home Care
Responsibilities:
- Assess, plan, implement, and evaluate patient care.
- Administration of medications and documentation
- Health promotion, teaching, and training of family members
- Wound care
- Patient assessments, coordination of care
- Monitor and report changes in patient symptoms or behavior
Qualifications:
- L.P.N, LVN or state-specific licensed nurse credential
- Must be at least 18 years of age
- Previous Home Care experience preferred, and a desire to serve and care for seniors.
- Must be licensed and in good standing (LPN) in the state of Delaware
- Minimum of six (6) months of experience working with seniors in assisted living, home health, independent living, hospital or long-term care environments and a desire to serve and care for seniors
- Ability to react and remain calm in difficult situations
- Ability to demonstrate good judgment, decision-making skills, and problem-solving.
- Possess written and verbal skills for effective communication and a level of understanding
- A driver's License and good access to transportation
- CPR
