You will be stepping into a physician-led, team-based environment where support is built into the model, patient care is coordinated rather than fragmented, and the system around you is designed to help you do your best clinical work, not just survive the day.

  • Managing patients with chronic and complex conditions

Requirements
  • NP or PA licensure; 3+ years of experience
  • Active Connecticut license or ability to obtain
