- Full-time, outpatient-only opportunity
- Monday-Friday
- 8:00a-5:00p
- No hospital responsibilities
- No nursing home responsibilities
- Outpatient geriatric practice
- Strong focus on value-based care
- Excellent patient volume with a maximum cap of 11 patients per day
- Managed care and/or value-based care experience preferred
- Prefer candidates with at least 1-2 years of post-training experience
- Office-based call rotation of 1:3 weekends
- Phone call only
- No in-person visits
- Call is limited to issues involving established office patients
- Practice is seeking a provider who wants to join the team and grow with the organization

- Base salary of $110,000-$125,000, based on experience
- Base salary reevaluated every two years
