Description of Work Assignment Day 8-5pm

Breakdown of Clinic Days/Sessions and/or OR Days Daily Angio or Cross Sectional

Schedule of Days/Times Needed to Work/ Shift Hours Day 8-5pm and Call 5pm-8am

On call? Yes

Patient Load 8-10 cases

Board Status Requirement (BC/BE) B/C

Is there additional support staff? no

Is the provider working solo or with other providers? If so, whom? Yes, another IR provider

Trauma Level of Work Location Level 1
