Family Medicine Physician Ohio

Responsibilities
- Provide preventive, acute, and chronic primary care services for patients across the lifespan
- Perform evaluations, physical exams, diagnostic assessments, and develop treatment plans
- Order, interpret, and act on laboratory and diagnostic testing
- Manage same-day and acute outpatient visits, including urgent-care-type presentations
- Coordinate referrals and collaborate with specialists as needed
- Maintain timely, accurate documentation in the EHR
- Partner with behavioral health, dental, nursing, and care-coordination teams
- Participate in quality improvement, population health, and community health initiatives
- Educate patients on prevention, wellness, and chronic disease management
Qualifications
- MD or DO from an accredited medical school
- Completion of an accredited Family Medicine residency
- Board Certified or Board Eligible in Family Medicine
- Family Medicine Obstetrics certification preferred, if applicable
- Active Ohio medical license or eligibility to obtain one
- Active DEA registration or eligibility
- Experience or strong interest in community health and underserved populations
- Strong communication, teamwork, and clinical documentation skills
- Commitment to culturally responsive, trauma-informed, patient-centered care
