  • Call Schedule: Rotating evening and weekend call

Responsibilities:- Practices evidence-based, cost-effective primary care in a team-based model.- Makes appropriate diagnoses and prescribes appropriate prescriptions and referralsQualifications:- Bilingual English/Spanish, required- Current IL State License, Controlled Substance License, and DEA Registration
