Compensation: - $310,000 - $368,500- Provider incentive bonus is performance-based, not RBU or fee-for-service

Additional Info: Hybrid role: Working from home, in the clinic, and visiting local practices. - Living in Jacksonville, Daytona Beach, or the Space Coast is requiredBecome an instrumental member of our primary care team. You will combine clinical practice with leadership and clinical management to ensure patient care and alignment with value-based care (VBC) principles. Qualifications:- MD/DO with Florida License- BC in Family Medicine, Internal Medicine, or Geriatric Medicine- 5 years of experience in outpatient practice, with clinical experience in primary care, senior health, and value-based care (VBC)- 3 years of experience leading clinicians in a leadership role with demonstrated impact on clinician talent, culture, and performance, partnership with operations, and supporting teams
