We are seeking a dedicated and experienced Medical Director ? Geriatric Medicine to join a well-established interdisciplinary team in West Springfield, MA. This full-time leadership role is based at a Program of All-Inclusive Care for the Elderly (PACE) facility, where the focus is on providing comprehensive, compassionate care to a frail elderly population. Schedule & Shift Details:

Responsibilities:
- Provide clinical leadership and oversight for the interdisciplinary care team
- Deliver direct patient care to a geriatric population within the PACE model
- Ensure high-quality, coordinated care across all settings including clinic, home, and long-term care facilities
- Participate in quality improvement initiatives and regulatory compliance
- Collaborate with administrative leadership to support program development and strategic planning
Qualifications:
- Board Certified in Geriatric Medicine or Internal Medicine/Family Medicine with significant geriatric experience
- Active Massachusetts medical license (required)
- MD or DO degree from an accredited institution
- Strong leadership and communication skills
- Experience working in interdisciplinary team settings preferred
