Registered Nurse (RN) Home Health Case Manager

Overview:
Our home health agency in Denton is seeking a Registered Nurse (RN) Case Manager to provide comprehensive patient assessments, care planning, and coordination for patients in their homes. This full-time role offers a consistent Monday Friday schedule with strong benefits and on-call incentive pay.
Position Offers:
- Schedule: Monday Friday, 8:00 AM 5:00 PM
- Competitive salary: $89,000 per year (DOE)

- Mileage reimbursement at $0.70 per mile
- On-call pay of $250 for every other weekend
Key Responsibilities:
- Conduct comprehensive patient assessments
- Develop and execute individualized care plans
- Coordinate care across the healthcare team
- Provide patient education
- Maintain accurate documentation and ensure compliance
- Perform basic wound care
Qualifications:
- Active, current Registered Nurse (RN) licensure in Texas
- Minimum 1 year of Home Health experience, with OASIS and EMR experience
- Valid driver's license and auto insurance required
