Conduct virtual clinical assessments for Medicare beneficiaries (aged 65+) and design Chronic Care Management (CCM) plans executed by a dedicated support team. Enjoy 100% remote flexibility, complete clinical autonomy, and zero administrative burden. Please note, this position does require multi-state licensure of 40+ states.
