• Clinic Volume: 20 patients/day

• Procedures: 2 dedicated procedure days/week all interventional pain cases

• Support: CRNA on all procedure days

• Team Structure: Incoming physician will be 1 of 2.5 physicians

• Setting: Outpatient only, Monday Friday, no nights/no weekends

• Year 1: Guarantee salary

• Year 2+: Transition to zero-cap productivity model
  

• Current physician earning: $800K

• Bonuses: Performance-based bonus

• Relocation: Up to $15K
