Great opportunity for a full-time nurse practitioner (NP) or physician assistant (PA) to join our team, providing primary care services in a skilled nursing facility located in Syracuse, New York.
Key Responsibilities
- Perform comprehensive patient assessments, including medical histories and physical exams
- Develop, implement, and adjust individualized care plans based on patient needs
- Monitor progress and coordinate care to optimize outcomes

Qualifications
