An established community healthcare organization is seeking a skilled Family Nurse Practitioner to deliver comprehensive primary and episodic care to a diverse patient population. This role blends preventive health services, chronic disease management, urgent care treatment, and patient advocacy within a collaborative, team-oriented environment. The ideal candidate is committed to improving access to care and supporting positive health outcomes for underserved communities.

Primary Responsibilities Direct Patient Care
  • Conduct comprehensive evaluations, health screenings, and focused assessments.
  • Diagnose and treat both acute and chronic medical conditions.
  • Develop individualized care plans based on patient needs and clinical findings.
  • Prescribe medications, therapies, and other interventions in accordance with state regulations and organizational protocols.
  • Address walk-in and same-day patient concerns while ensuring timely access to appropriate treatment.
Care Coordination
  • Collaborate with physicians, nurses, behavioral health professionals, and support staff to provide integrated care.
  • Coordinate referrals to specialty services, diagnostics, and community resources when necessary.
  • Ensure continuity of care through effective follow-up and patient outreach efforts.
  • Assist patients in navigating available healthcare programs and support services.
Health Promotion & Education
  • Counsel patients on disease prevention, wellness strategies, medication adherence, and lifestyle modifications.
  • Provide guidance on managing chronic illnesses and improving overall health outcomes.
  • Participate in educational programs, health awareness initiatives, and community engagement activities.
Documentation & Quality
  • Maintain accurate and compliant clinical records within the electronic health record system.
  • Adhere to evidence-based practice standards and regulatory requirements.
  • Support quality improvement initiatives and population health goals.
  • Contribute to a patient-centered environment focused on safety, dignity, and respect.

Qualifications Required
  • Master's Degree in Nursing from an accredited institution.
  • Current Nurse Practitioner license in good standing.
  • Board certification as a Nurse Practitioner.
  • Active DEA registration.
  • Minimum of two years of advanced practice experience.
  • Demonstrated experience caring for patients affected by HIV.
  • Strong clinical decision-making and assessment skills.
  • Effective communication and relationship-building abilities.
  • Proficiency with electronic medical records and healthcare technology platforms.
Preferred
  • Family Nurse Practitioner certification.
  • Specialized training or certification in HIV-related care.
  • Experience practicing within community-based, ambulatory, or safety-net healthcare settings.
  • Bilingual skills in Spanish, French, Haitian Creole, or other commonly spoken languages.
  • Experience working with culturally diverse and medically underserved populations.
