Opportunity:Advanced Practitioners - Physician Assistant - Geriatrics
Job Details:- Full-time Nurse Practitioner or Physician Assistant position supporting the home-based REACH (Resource for Elderly and Care at Home) Program
- Open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA)
- Deliver compassionate geriatric care in patients' homes and assisted living facilities in the greater Manchester, CT area
- Work within a multi-disciplinary care team focused on Geriatric Medicine in the home setting
- Monday through Friday schedule with no weekend requirements
- Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients
- Develop plans for further evaluation of patient health needs, including referrals to specialists and other health team members
- Provide appropriate treatment for problems within the scope of practice and refer appropriately when needed
- Unrestricted State of Connecticut Medical License required (or eligible)
- Minimum of two years of post-graduate experience as a Nurse Practitioner or Physician Assistant in Geriatric Medicine or a related field required
