Tele-Pediatric Nephrologist
Part-Time Flexible Schedule Work from Anywhere Rural Texas
Position Overview
The Rural Health Hub (RHH) is seeking a Tele-Pediatric Nephrologist to provide high-quality kidney and hypertension care to children and adolescents in rural and medically underserved Texas communities.
This flexible, part-time role allows the pediatric nephrologist to work remotely while trained clinical staff are physically present with the patient. The physician will evaluate and manage a broad range of pediatric renal conditions, provide longitudinal specialty care, and collaborate with families, local primary care providers, hospitals, laboratories, urologists, dietitians, and other specialists.
The Rural Health Hub Model
RHH uses a hybrid-telehealth model that combines remote specialty expertise with in-person clinical support.
Local medical assistants, nurses, or advanced practice providers assist with:
History, medications, vital signs, growth measurements, and symptom assessment
Accurate blood pressure measurement and trending
Height, weight, BMI, and growth-curve documentation
Physical examination components, including assessment for edema
Connected examination technology when appropriate
Collection and coordination of blood and urine testing
Coordination of renal ultrasound, imaging, and other diagnostic studies
Medication reconciliation and adherence support
Referrals, follow-up care, and communication with the local care team
This model allows pediatric nephrologists to provide meaningful ongoing specialty care while using local and regional resources for laboratory testing, imaging, procedures, hospitalization, dialysis, and higher levels of care when needed.
Key Responsibilities
Conduct initial and follow-up pediatric nephrology visits by telehealth
Evaluate and manage common pediatric kidney and hypertension conditions
Develop individualized diagnostic and treatment plans
Review blood pressure trends, growth data, laboratory results, urinalysis, imaging, and prior medical records
Order appropriate blood tests, urine studies, imaging, and referrals
Provide longitudinal medication and disease management
Monitor renal function, electrolyte abnormalities, and progression of kidney disease
Educate and collaborate with patients, parents, and caregivers
Identify patients requiring urgent, procedural, or in-person evaluation
Coordinate care with pediatricians, family physicians, hospitals, urologists, dietitians, and other specialists
Complete timely documentation and clinical follow-up
Clinical Scope
Patients may include children and adolescents with:
Pediatric hypertension and elevated blood pressure
Proteinuria
Hematuria
Recurrent urinary tract infections
Chronic kidney disease
Acute kidney injury follow-up
Congenital anomalies of the kidney and urinary tract
Hydronephrosis
Vesicoureteral reflux
Nephrotic syndrome
Nephritic syndromes and glomerulonephritis
Electrolyte abnormalities
Acid-base disorders
Renal tubular disorders
Kidney stones and nephrolithiasis
Hypercalciuria
Cystic kidney disease
Hereditary and genetic kidney disorders
Renal complications of systemic diseases
Abnormal renal function or urine findings requiring specialty evaluation
Long-term monitoring following significant renal illness or hospitalization
Patients requiring dialysis, renal biopsy, surgery, transplantation, or other advanced interventions would be coordinated with appropriate regional pediatric nephrology and hospital-based resources.
Qualifications
MD or DO degree
Board certified or board eligible in Pediatric Nephrology
Active Texas medical license or eligibility to obtain one
Strong pediatric nephrology diagnostic and management skills
Ability to practice effectively through telehealth
Strong communication skills with children, adolescents, and families
Strong collaboration skills with local clinicians and care teams
Commitment to rural and underserved populations
Experience in telemedicine, rural healthcare, pediatric hypertension, chronic kidney disease, congenital renal disorders, nephrotic syndrome, or other pediatric nephrology subspecialty care is welcomed but not required.
