Seeking a board certified or truly board eligible physician in Geriatrics, Internal Medicine, Palliative Care/Hospice, or Family Medicine
Join the nursing home division of a large multispecialty group
Opportunities in the Bronx, Brooklyn, Poughkeepsie, and Westchester County
Provide primary care to patients in a combination of subacute and long-term care settings
Schedule: Monday-Friday, 9:00 a.m.-5:00 p.m.
Some facilities have 12-hour shifts; specific schedules to be discussed
Set onsite hours are required; this is not a round-and-go position
Notes may be completed at home

Credentialing typically takes 2-3 months

Clinical Responsibilities:
Provide comprehensive primary medical care to nursing home patients
Conduct patient assessments and review medical histories, laboratory results, and imaging
Diagnose conditions and develop individualized treatment plans
Prescribe medications and treatments while considering allergies and potential side effects
Provide nutrition and lifestyle guidance to support patient health and disease prevention
Collaborate with nursing staff, specialists, and the primary care team
Refer patients to specialists when appropriate
Participate in quality improvement, patient safety, and infection control initiatives
Maintain complete, accurate, and timely documentation in eClinicalWorks
Follow applicable standards of care and patient privacy requirements
Qualifications:
Board certified or truly board eligible in one of the specialties listed above
MD or DO degree from an accredited U.S. medical school
Completion of an accredited residency program or the specified U.S. residency/five-year pathway
Active New York State medical license
DEA registration and any applicable controlled-substance registrations; may be obtained during credentialing
Clinical experience in primary care and diagnostics within a team-based setting
EMR experience; eClinicalWorks preferred
Strong communication, interpersonal, and clinical judgment skills
Compensation & Benefits:
Annual salary: $190,000-$240,000
