Primary Care Clinician (MD, NP, or PA) - In-Home Assessments
Location: Florida
Position Overview
Conduct in-home health and wellness assessments for health insurance plan members.
Typical responsibilities include history and physical evaluations, blood pressure checks, finger stick glucose testing, urinalysis collection, and distribution of fecal immunochemical test kits.
Patient population: geriatrics.
Work is performed independently in members' homes within assigned counties.
Documentation is completed in a proprietary iPad-based system.
Schedule
Monday through Sunday, 7:00 AM to 7:00 PM.
Typical workload: 5 to 8 assessments per day (assessments generally last 45 to 75 minutes).
Must have at least one weekday available (weekend-only availability is not accepted).
Required Qualifications
Active, unrestricted Florida license as an MD, NP, or PA.
National certification required for NP or PA candidates.
NPI required (CAQH may be obtained during credentialing if needed).
Must be willing to travel (minimum 75 miles from home address) and cover a minimum of 3 counties.
Must carry personal auto insurance and meet driving record requirements.
Candidates must have at least two years of clinical experience within the past five years to qualify.
