• Start Date: As soon as credentialed
• Shift/Schedule Details: 4, Day (10 Hours) 7:30 - 16:30
• Credentialing Timeframe: 6-8 weeks typically
• General Requirements:
• Board Certification: BC in Hem & Onc
• License Required: AK
• DEA: yes
  
• Support Staff: 3 FTE MD s, 1 FTE NP, 1 per diem NP
• Volume/# of patients per day: 12-13 patients per day 1hr new consults 30 mins follow ups
