Job description

Consider this new opportunity to serve as an Outpatient Medical Director for a Critical Access Hospital in Northern Arizona. Based on referral volumes and community need, the incoming provider should be full quickly.
  • Serve as Supervising Provider for the Patient Centered Medical Home program
  • 50% clinical and 50% Administrative
  • Provide direct supervision of medical staff members assigned to the outpatient departments
  • Reports to Chief Medical Officer
  • At least 3 years experience as a Medical Supervisor in an outpatient clinic setting
  • Strong familiarity with the Patient Centered Medical Home model of care and understanding in basic concepts in Population Health Management.
This region of Arizona is at a higher elevation, so it is not as hot as other parts of the state. Average high in the summer is 85. There are beautiful canyons and rolling hills. Enjoy easy access to Flagstaff, Durango (CO) and to Monument Valley!
