About the Practice
You'll provide comprehensive outpatient primary care for senior adults in a modern, relationship-driven environment focused on preventive medicine and chronic disease management.
Unlike traditional fee-for-service practices, this organization emphasizes meaningful physician-patient relationships, proactive healthcare, and improved long-term outcomes.
You'll have the autonomy to practice medicine while receiving exceptional clinical, operational, and technological support.
What You'll Do
Deliver comprehensive primary care services to Medicare Advantage and senior patients
Build long-term relationships with patients and their families
Diagnose and manage acute and chronic medical conditions
Develop individualized treatment plans
Focus heavily on preventive care and wellness initiatives
Manage chronic diseases including:
Diabetes
Hypertension
COPD
Congestive Heart Failure
Chronic Kidney Disease
Coordinate care with specialists when appropriate
Utilize EMR tools and quality dashboards to improve patient outcomes
Collaborate with advanced practice providers, pharmacists, care coordinators, and support staff
Participate in value-based care initiatives focused on quality metrics and patient satisfaction

Qualifications
Required
