Job/Practice Description: Inpatient Palliative Care NP - Full-time inpatient position: 75% at one hospital in Findlay, OH and 25% at a second hospital in Lima, OH
- Monday Friday schedule, 8:00 AM 4:30/5:00 PM
- Shared evening and weekend telephone call rotation
- Perform inpatient palliative care consultations and follow-up visits
- Manage symptoms for patients with serious, chronic, and life-limiting illnesses
- Facilitate goals-of-care discussions and advance care planning with patients and families
- Assist with discharge planning, including transitions to home-based palliative care or hospice
- Provide inpatient rounding for hospice General Inpatient (GIP) patients at the second hospital as needed
- Professionals with a passion for palliative care encouraged to apply
Requirements: - Certified Nurse Practitioner (Acute Care NP preferred; palliative care or hospice experience welcomed)
- No experience needed!
- Strong communication, interpersonal, and organizational skills
- Comfortable navigating complex conversations related to serious illness, prognosis, and end-of-life planning
- Ability to work independently while collaborating within an interdisciplinary team
